
We are currently accepting new patients, and it would
be an honor to serve you! Patients are seen by appointment
only, so please call in advance and reserve a time for
your child with our staff. If you would like to schedule
an appointment, please feel free to call our office
at 317-773-3617, and we will
gladly dedicate a special time for you to meet with
Dr. Forgey or Dr. Hazelrigg for a consultation. We make
every effort to be on time for our patients, and we
realize that your time is as important as ours. Therefore,
we ask that you extend that same courtesy to us.
If, for some reason, you cannot keep your scheduled
appointment or will be delayed, please call us as soon
as possible. No charge will be made for rescheduled
appointments provided at least 24 hours notice is given.
This will allow the time reserved for your child to
be made available to other patients. If you have a personal
emergency, please let us know as soon as possible.

Teamwork among the patient, parent and doctor is essential
for excellence in dental care. We anticipate this from
you and your child, and it is our goal to render the
best dental care available. We would appreciate your
discussing any questions or concerns with us at any
time. Feel free to call our office at any time, and

In preparation for your child’s first visit at
our office we would recommend trying to make dental
visits an important adventure for you child. Tell him/her
the dentist is a friendly doctor who will help him/her
keep their teeth and mouth healthy. Talk about the visit
in a positive, matter-of-fact way as you would any new
experience. A good first trip will help mold your child’s
feelings for many years.
We
have made our forms available to you online. The form
below MUST be completed prior
to meeting with Dr. Forgey or Dr. Hazelrigg at your
child’s first dental visit. To save time at your
first appointment, we would appreciate it if you would
print the forms, complete them, and bring them with
you at your child’s first dental appointment.
By doing this, time will be saved at your first visit
Pre-medication Needs:
Certain medical conditions require an antibiotic to
be taken prior to dental care. If your child’s
physician has informed you that pre-medication is required,
please let us know. We adhere to the American Heart
Association guidelines for pre-medication and want to
provide the precautions necessary for each child’s
specific health care needs.
